An early look at the fourth episode of “The Walking Dead” is a bit of a jarring visual shock for long-time fans. Emily Kinney’s Beth Greene has been missing since last season, when she was abducted and taken away from Daryl (Norman Reedus).

In the clip, which you can see above, she awakens in what looks like a sterile and clean hospital with a ticking clock; a sound she probably hasn’t heard in a long time.

A peek out of the window makes it clear this is still a post-apocalyptic world, as it shows a ravaged cityscape. But still, Beth is in a hospital with an IV drip, bandaged and looking very disoriented.

Things only get stranger when a doctor and a police officer come in, trying to both calm and control her.

“Where am I?” she asks. But that question isn’t going to be answered in a teaser.

“The Walking Dead” continues on Sundays at 9 p.m. ET on AMC.
